Multidrung-Resistant Tuberculosis in Previously Treated Patients : Duration from Completion of First Treatment to Starting Retreatment

Abstract
The prevalence of pulmonary tuberculosis decreased remarkably for the past 30 years, but pulmonary tuberculosis is an important infections disease yet in Korea. In retreatment of tuberculosis, the drug-resistant and multidrug-resistant tuberculosis(MDR-TB) are high. So prognosis of retreatment is poor. The aim of this survey is to discover a relation between duration from completion of first treatment to starting retreatment and rate of MDR-TB infection in previously treated patients. The datas from two publich health centers in Kwanjin-Ku, Seoul and Chungju from
April 1994 to September 1996 were reviewed. The inclusion criterias were age, sex, completion period of first treatment, retreatment starting period, AFB culture and drug sensitivity test result and 46 patients were met in these criterias.
Of the 46 patients, AFB culture and drug sensitivity were revealed MDR-TB in 18 patients. The average durations from completion of first treatment to starting retreatment were 25.0 months in patients who revealed MDR-TB and 99.2 months in patients who did not. Discriminant analysis reveal no difference in age, but significant difference in duration between MDR-TB and non MDR-TB groups. The shorter the duration from completion of first treatment to starting retreatment, the higher the rate of MDR-TB infection. Also, the MDR-TB infection accounts for 75% of patients to whom pulmonary tuberculosis reccured within 3 years and 9% after 4 years.
